Ekstrak bunga telang memiliki sumber antioksidan tinggi yang bermanfaat untuk kecantikan kulit wajah yang dapat dijadikan masker sheet untuk mengatasi penuaan dini. Tujuan penelitian untuk mengetahui kualitas sediaan masker sheet ekstrak bunga telang berdasarkan uji organoleptik, uji laboratorium yang meliputi uji antioksidan dan uji pH, dan uji klinis. Metode penelitian menggunakan eksperimen One Group Pre Test-Post Test. Teknik pengumpulan data menggunakan observasi dan uji laboratorium. Analisis data menggunakan analisis deskriptif presentase. Hasil penelitian berdasarkan uji organoleptik terhadap ketiga formula produk masker sheet ekstrak bunga telang dinyatakan sangat layak dengan presentase terbanyak ada pada formula 3 yang memiliki presentase 100%. Hasil pengujian laboratorium ketiga formula sediaan masker sheet ekstrak bunga telang memiliki pH 6 yang artinya aman, serta memiliki kandungan antioksidan terbaik ada pada formula 3, dengan hasil 63,6615%. Hasil uji klinis ketiga formula terdapat pengaruh yang signifikan sebelum dan sesudah perawatan terhadap tingkat kelembapan kulit wajah. Skor unggul didapat pada formula 2 dan formula 3, pada aspek kelembapan dahi dan dagu formula 2 lebih unggul sedangkan terhadap aspek kelembapan pipi kanan, pipi kiri, dan hidung formula 3 lebih unggul. Uji klinis iritasi terhadap responden yang mendapat perlakuan dari ketiga formula masker sheet ekstrak bunga telang menunjukkan kriteria tidak menimbulkan iritasi dengan nilai akhir 100%. Simpulan ketiga formula masker sheet ekstrak bunga telang berdasarkan uji organoleptik sangat layak, uji laboratorium dari segi pH aman dan memiliki kandungan aktivitas antioksidan, dan uji klinis ketiga formula terdapat pengaruh yang signifikan sebelum dan sesudah perawatan terhadap tingkat kelembapan kulit wajah.
