Abstract

A comunicação de más notícias representa um desafio aos profissionais que atuam em cuidados paliativos, com entraves que precisam ser compreendidos. Para descortiná-los, objetivou-se analisar a perspectiva de médicos que atuam em cuidados paliativos com adultos acerca da comunicação de más notícias. Realizou-se uma pesquisa qualitativa, que contou com a participação de nove médicos que trabalham em cuidados paliativos, em hospitais públicos e privados, com experiência na especialidade entre 2 e 18 anos. Responderam a um roteiro semiestruturado, contendo as seguintes temáticas: comunicação de más notícias, capacitação profissional e dificuldades dessa prática. Os dados foram analisados no software IRaMuTeQ. Os resultados organizaram-se em cinco classes, que apresentam: 1) a importância e as deficiências na capacitação e prática dos profissionais; 2) os cuidados necessários no processo de comunicação; 3) a reação do paciente e de seus familiares diante do diagnóstico e prognóstico em cuidados paliativos; 4) a integração entre equipe e família em cuidados paliativos e; 5) a importância de verificar a compreensão dos pacientes sobre a comunicação. Conclui-se que a comunicação de más notícias em cuidados paliativos é um desafio que requer conhecimento teórico e habilidades em Psicologia Médica e da Saúde, que precisam ser enfatizadas na formação profissional.
