Abstract

Latar Belakang: World Health Organization (WHO) melaporkan pada tahun 2024 terdapat 287.000 kematian ibu di seluruh dunia, setara dengan 800 kematian per hari atau satu kematian setiap dua menit. Angka ini masih sangat tinggi, terutama akibat perdarahan postpartum, hipertensi dalam kehamilan, dan komplikasi obstetri lainnya. Salah satu kelompok risiko tinggi adalah grandemultipara (paritas ≥5), yang rentan mengalami inersia uteri, atonia uteri, serta perdarahan postpartum akibat perubahan fisiologi uterus setelah persalinan berulang. Upaya promotif dan preventif diperlukan untuk menekan angka kematian ibu, salah satunya melalui edukasi dengan media sederhana seperti kalender kesehatan. Tujuan: Mendeskripsikan asuhan persalinan normal kala I–IV pada ibu grandemultipara dengan pemberian edukasi menggunakan kalender kesehatan. Metode: Penelitian ini merupakan studi kasus deskriptif dengan pendekatan SOAP. Subjek adalah Ny. E, 29 tahun, G6P5A0, hamil 39 minggu, yang menjalani persalinan di TPMB Leni Triyana, S.Keb., Bdn Kota Banjarmasin pada 19 Juni 2025. Data dikumpulkan melalui anamnesis, pemeriksaan fisik, observasi proses persalinan, serta dokumentasi partograf. Hasil: Persalinan berlangsung normal pada kala I–IV tanpa komplikasi. Edukasi dengan kalender kesehatan diberikan selama proses asuhan dan terbukti meningkatkan pemahaman ibu mengenai tanda bahaya kehamilan, risiko perdarahan postpartum, serta pentingnya perawatan pascapersalinan. Kesimpulan: Asuhan persalinan normal kala I–IV pada ibu grandemultipara dapat berjalan sesuai standar pelayanan kebidanan. Media kalender kesehatan efektif digunakan sebagai sarana edukasi untuk meningkatkan kesadaran ibu terhadap risiko persalinan berulang serta pencegahan komplikasi.

This intervention reportedly enhanced the mother's understanding of danger signs, postpartum hemorrhage risks, and the importance of postnatal care. The study concludes that standard midwifery care for grandemultipara can be achieved, and the health calendar serves as an effective educational tool for increasing maternal awareness. While the study offers a valuable illustration of successful care for a high-risk individual and demonstrates the potential utility of a simple educational tool, its primary limitation lies in its descriptive case study design with a single subject. This significantly restricts the generalizability of the findings, making it challenging to draw broad conclusions about the effectiveness of the calendar or the typical outcomes for grandemultipara populations. Future research would benefit immensely from employing larger sample sizes, perhaps comparative studies to objectively quantify the impact of the health calendar against standard education, or even a mixed-methods approach to delve deeper into the mechanisms of improved maternal understanding and its correlation with clinical outcomes. Despite this limitation, the study provides an encouraging case example and a foundation for further, more robust investigations into scalable educational interventions for high-risk obstetric populations.
