Introduction

Parodontális vertikális csontdefektusok kezelése vérlemezkében gazdag plazmagéllel (prg), illetve nyitott küret technikával (ofd). Fedezze fel a parodontális csontdefektusok kezelését PRG és OFD technikákkal. Egy év után a PRG jelentősebben javította a szondázási mélységet és tapadási szintet.

Abstract

Az esetismertetések célja a parodontális intraosszeális csontdefektusok gyógyulásának bemutatása és klinikai értékeléseautológ vérlemezkében gazdag készítmény alkalmazása után (Platelet-Rich Gel, PRG), összevetve egy hagyományoskezelési módszer, a nyitott küret (OFD) eredményeivel.A vizsgálatban 28 intraosszeális defektussal rendelkező, nem dohányzó, krónikus parodontitisben szenvedő páciensvett részt. A tesztcsoport esetében nyitott küret technika (OFD) alkalmazása után PRG-t helyeztünk a defektusba, a kontrollcsoportbancsak OFD-t alkalmaztunk. Az öt bemutatott eset ezen kontrollált klinikai vizsgálat résztvevője. A kiértékelésegy évvel a műtétet követően történt a következő paraméterek rögzítésével: szondázási mélység (PD – probingdepth) és klinikai tapadási szint (CAL – clinical attachment level). Teljes vastagságú lebeny képzését követően a defektusküretálása, illetve gyökérsimítás történt. A tesztcsoportban vérlemezkében gazdag plazma (PRP) preparálása után,Ca-glükonát és friss vénás vér hozzáadásával vérlemezkében gazdag plazmagél (PRG) készült. Egy évvel a műtét utánmindkét technika esetén javulást tapasztaltunk, de a PRG csoportban jelentősebb szondázási mélység (PD) csökkenésés klinikai tapadási nívó (CAL) növekedés mutatkozott.

This abstract presents a study comparing the efficacy of autologous Platelet-Rich Gel (PRG) combined with Open Flap Debridement (OFD) versus OFD alone for the treatment of periodontal intraosseous bone defects. The primary objective was to clinically evaluate and demonstrate the healing of these defects. The investigation involved 28 non-smoking patients diagnosed with chronic periodontitis, each presenting with intraosseous defects. The abstract, however, specifically highlights and details five case reports drawn from this larger, controlled clinical investigation. The methodology involved a test group where OFD was followed by the application of PRG into the defect, while the control group received OFD alone. Clinical evaluation was performed one year post-surgery, assessing key parameters such as probing depth (PD) and clinical attachment level (CAL). The findings indicate that both treatment modalities resulted in improved periodontal conditions. Crucially, the PRG group demonstrated a significantly greater reduction in probing depth and a more substantial gain in clinical attachment level compared to the OFD-alone control group, suggesting a regenerative benefit associated with the application of PRG. While these preliminary results are promising and suggest that PRG holds potential as a valuable adjunct in regenerative periodontal therapy, a significant limitation lies in the abstract's presentation of only five case reports from the larger 28-patient controlled clinical investigation. To draw more definitive conclusions and fully substantiate these findings, the complete publication of the comprehensive 28-patient trial data is warranted. Future research should aim to present the full dataset, ideally expanding to larger patient cohorts and longer follow-up periods, to confirm the long-term stability and broader clinical significance of PRG in the challenging field of periodontal regeneration.
